What is Support Coordination?

Support Coordination is designed to help NDIS participants implement and manage the supports outlined in their NDIS plans. It is carried out together with and on behalf of participants and involves identifying, coordinating and accessing supports and services that meet your specific needs, wants and preferences.

How can Support Coordination help?

Support Coordination is a service which helps you to:

  • Understand and use you NDIS plan to work towards your goals
  • Connect you with NDIS providers, community, and other government services of your choice
  • Assist in managing your NDIS plan budget
  • Track the progress of your plan and prepare for your review meeting
  • Boost your confidence and skills to use and coordinate your supports

Our Support Coordinators are experienced in understanding the specific needs of people with any type of disability. We can help you to secure the right type of funding in your NDIS Plan and support you in choosing the best services for your needs.

Can I access Support Coordination services?

If you already have an NDIS plan, you will need to have 'Support Coordination' approved in the 'Capacity Building' budget.


Evolution Disability Services provides services for Level 2: Coordination of Supports only.


If you haven't got support coordination approved in your NDIS plan and feel you may benefit from accessing this service, it is important to highlight the reasons why to your Local Area Coordinator (LAC) or NDIS Planner.


Participants may receive funding for support coordination in their NDIS plan depending on their individual circumstances. The NDIS may approve support coordination funding for participants who:

  • are new to the NDIS and are receiving an NDIS Plan for the first time
  • have limited or no informal supports (e.g. family, carers, friends) to assist them in navigating the NDIS
  • are likely to develop the skills and confidence needed to navigate the NDIS independently
